Update Personal Details

To update your personal details, such as address or employer, please complete the attached form in full. We require any change of details to be made in writing and cannot make changes over the phone. The form can be returned by email, post or fax to the details below.

If you are changing your name due to marriage or deed poll, please include a photocopy of the relevant certificate. Do not send originals. If you require a new card a replacement card fee will be charged.
