Replacement Cards

If you have an Energy & Utility Skills Register (EUSR) card with a valid endorsement a replacement card can be obtained by completing the attached form and returning it to EUSR by post or email.

Please complete this in full as it will allow us to update our system with your current details and ensure that your replacement card reaches you in a timely fashion.

Replacement cards are charged at £10 (+VAT at the current rate) each. Payment can be made by cheque or company purchase order. For details on payment methods please consult our payment guidelines page. We cannot accept payment by cheque for emailed requests.
